The New York Times shared details:
The national proof of vaccination standard features a QR code and official logos from the federal government and the recipient’s province or territory. It will be available digitally and in hard copy.
Some of the country’s 10 provinces and its three territories, which are responsible for delivering health care, now offer digital vaccine cards with scannable QR codes and paper copies; others offer paper cards that can be digitized or displayed in phone photos. Regulations about what residents may be asked to present when they enter places like restaurants and movie theaters vary from province to province.
So far, five provinces — Saskatchewan; Ontario; Quebec; Nova Scotia; and Newfoundland and Labrador — as well as the Yukon, Nunavut and Northwest Territories have implemented what will now be the national standard, Mr. Trudeau said, adding that the remaining five provinces were expected to catch up soon.

He added that the federal government will be “picking up the tab” for this standardized certificate. Canadians will not be able to board a plane for foreign or domestic travel without a the certificate starting on November 30.
The information on the proof of vaccination includes:
- Full name and date of birth
- A “neutral, factual” account COVID-19 vaccination history, including the dates of vaccination, the number of doses, vaccine type(s), product name(s) and lot number(s)
- A QR code that includes the vaccination history and may include “additional information on the COVID-19 vaccines received”
The federal government has indicated that this proof of vaccination complies with the SMART Health Card standard, recognized by a number of international travel destinations.

While enforcement begins October 30th, Canadians have a one month grace period to use negative COVID-19 tests to qualify for travel.
Starting November 30th, the negative test option goes away.
And only proof of COVID-19 jab permits Canadians to travel inside their own country by air, rail, or cruise ship.
